: 1.) A copy of your child's Baptism certificate must be submitted to the Religious Education Office by December 1, 2020 in order for your child to participate in any of the sacraments. No exceptions. If your child was baptized at St. Matthew's and you need a copy of your baptismal certificate, please contact Kristy Hayes at (904) 388-8698 in St. Matthew's Business Office
: 2.) All candidates for sacramental preparation must have received two (2) consecutive years of Religious Education.
: 3.) This form must be returned or submitted to the Religious Education Office by December 1, 2020 with a $15.00 book fee and a copy of your child's Baptism certificate.
